Given out FREE to patients attending the practice Designed as a take-home booklet. Contains Patient Medical Record forms for patient use, plus the recommended Immunisation Schedule, and if space permits, health tips on cholesterol control, stress control or First Aid. The practice’s contact details are on the front cover, with emergency and hospital numbers on the back; 1400 copies sent to the practice for distribution to patients.

We offer those placing an advertisement:

	A twelve-month advertisement in a take-home booklet designed as a handy reference for patients;
	A free editorial to further describe your services or specialties, and to reinforce any offers in your ad;
	Assistance with your ad and editorial;
	An opportunity (optional, no costs involved) to put your contact details and links (email and/or website address) on our website while you remain a current advertiser with any of our printed guides;
	No payment due until guides are printed and in surgery.

We offer those placing an entry in the Locality Directory:

	A chance to put your contact details in front of patients at the surgery at a minute price;
	No payment due till guides are printed and in surgery.

ACCEPTANCE OF AD

Once you have accepted this offer, you can be sure:

	Graphic artist will take care of your ad layout, using your logo, graphics and text
	If you have your own special ad or ad layout ideas, we will work with you to follow your requests
	If you need assistance with the text for your editorial, we will help you.
	A final proof will always be sent to you for your signed or emailed approval before your ad and editorial are sent to the printer.
